2018-2019 Student-Athlete Medical Check In Procedures and Forms

All required paperwork must be submitted and cleared by the Head Athletic Trainer prior to any physical involvment with the Lesley Univeristy Athletic Department's Athletic Programs. 



The Lesley University Athletic Department along with Cambridge Health Alliance Sports Medicine is dedicated to providing the best possible sports medicine treatment for all athletes.  Through the use of the athletic training room at Lesley University, rehabilitation equipment, Medford High School as well as the facilities at Buckingham Browne & Nichols, our athletic trainer serves the nearly 140 student-athletes on campus.
In accordance with the goals and values of Lesley University and the Department of Athletics, the staff will provide comprehensive sports medicine care to all varsity athletes at the university.  This service is critical to assist student-athletes in safely reaching athletic and personal goals through each sport.  The sports medicine program will also serve as a source of information to athletes, coaches and families in many aspects of the overall health and wellness of student-athletes.
The sports medicine staff will treat each athlete on an individual basis with emphasis on injusry prevention and safe return to participation.  Individuals are guaranteed fair and equal treatment without regard to factors such as, but not limited to, race, gender, sexuality, religion or ability.  It is the goal of the sports medicine staff to facilitate immediate and long-term wellness of its athletes.
